All application materials for the MS program can be downloaded from this website. If you want to have an application packet mailed to you, please contact the the director of student services.

The application materials for the MS program include the following:

• Application checklist
• Résumé or CV
• Financial aid/General information questionnaire
  
• Official Transcripts
• Statement of educational goals
• Three letters of recommendation (optional evaluation form for recommendations)
• UW Graduate School online application
